• If you have an existing, approved facility permit and need to make changes, please use this form.
  
• If your facilities request is still pending approval and you need to make changes, you can make changes directly in your facility account.

• While waiting for your user account to be created, please feel free to fill out this application and email it to facilityrentals@slpcommunityedschools.org.
